These are forms that can be downloaded for the host to use in the registration packets. These forms such as the name tag form, daily registration form and registration form for packet can be downloaded by the departments (members) once they are updated each year for the host’s information and costs. These are Tri-State Official forms and hosts may use them as needed when organizing their member packets for attendees of the convention/training school each year. Just click on the name of the form that you want, there is a link box that has ts-fa.org in it, click on that and it will redirect you to another page where you click on the attachment link and the form will download so that you can save it on your host computer.
